Agreement to Terms

These Terms of Service ("Terms") govern your access to and use of the website wellnessforlove.com and the mobile IV therapy and concierge wellness services ("Services") provided by Wellness for Love, a DBA of Loved One Care, Inc., a California corporation (EIN: 85-0872039, NPI: 1104447234). By accessing our website or booking our Services, you agree to be bound by these Terms. If you do not agree, please do not use our website or Services.

About Our Services

Wellness for Love provides mobile intravenous (IV) therapy, vitamin injections, and concierge wellness services delivered by licensed Registered Nurses (RNs) to your home, office, hotel, or event location. All clinical services are performed under the medical oversight of our Medical Director, Dr. Tej Mudigonda, MD. We serve Los Angeles County, Orange County, and Riverside County, California.

Eligibility

You must be at least 18 years of age and in general good health to use our Services. By booking an appointment, you represent that you meet these requirements. Certain medical conditions may disqualify you from receiving treatment. Our clinical team will evaluate your eligibility during the intake and screening process, and we reserve the right to decline or discontinue service at any time for clinical safety reasons.

Medical Services Disclaimer

Our Services are wellness services and are not a substitute for professional medical advice, diagnosis, or treatment from your primary care physician or specialist. The information provided on our website and during treatment is for general informational and wellness purposes only. You should always seek the advice of a qualified healthcare provider with any questions regarding a medical condition. These statements have not been evaluated by the FDA. Our services are not intended to diagnose, treat, cure, or prevent any disease.

Informed Consent

Prior to receiving any treatment, you will be required to complete a medical intake form and sign an informed consent document. These documents disclose the nature of the treatment, potential risks and side effects, and alternative options. IV therapy carries inherent risks including but not limited to bruising, infection at the injection site, vein inflammation, and allergic reaction. By signing the consent form, you acknowledge that you understand these risks and voluntarily agree to proceed.

Appointments, Payments, and Cancellations

Appointments are booked through our website, by phone, or via SMS. A reservation deposit may be required at the time of booking and will be applied to your total treatment cost. Payment is processed securely through Stripe. All prices are listed in U.S. dollars and are subject to change without notice.

SMS / Text Messaging Program

By providing your mobile phone number to Wellness for Love, you consent to receive text messages relating to your appointments and inquiries.

Program: Wellness for Love sends customer-care and account-notification messages, including appointment confirmations, scheduling and rescheduling, visit reminders, nurse dispatch and arrival updates, replies to your questions, and post-visit follow-up.

Message frequency: Message frequency varies based on your appointment activity, typically 1-5 messages per visit.

Cost: Message and data rates may apply, depending on your mobile carrier and plan.

Opt-out: You may cancel at any time by replying STOP, UNSUBSCRIBE, END, or QUIT to any message. You will receive a confirmation and no further messages.

Carrier disclaimer: Wireless carriers are not liable for delayed or undelivered messages. Messaging is supported on major U.S. wireless carriers.

User Responsibilities

When using our Services, you agree to:

• Provide accurate, complete, and current health and personal information
• Disclose all relevant medical conditions, allergies, and current medications
• Follow all pre-treatment and post-treatment instructions provided by our nursing staff
• Notify us immediately of any adverse reactions following treatment
• Ensure a safe, clean, and accessible environment for in-home or on-location treatments
• Be present and available at the agreed-upon location at the scheduled appointment time

Intellectual Property

All content on this website, including but not limited to text, graphics, logos, images, videos, and software, is the property of Wellness for Love / Loved One Care, Inc. and is protected by United States and international copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from any content on this site without our prior written consent.

Privacy

Your use of our website and Services is also governed by our Privacy Policy, which describes how we collect, use, and protect your personal and medical information. By using our Services, you consent to the practices described in that policy.

Limitation of Liability

To the fullest extent permitted by law, Wellness for Love, Loved One Care, Inc., and their officers, directors, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our website or Services. Our total liability for any claim arising from or related to a specific service shall not exceed the amount you paid for that service.

You agree to indemnify, defend, and hold harmless Wellness for Love, Loved One Care, Inc., and their officers, directors, employees, and agents from and against any claims, liabilities, damages, losses, or expenses (including reasonable attorneys' fees) arising from your breach of these Terms, your use of our Services, or your provision of inaccurate health information.

Governing Law and Dispute Resolution

These Terms shall be governed by and construed in accordance with the laws of the State of California, without regard to its conflict of law provisions. Any dispute arising from these Terms or your use of our Services shall be resolved exclusively in the state or federal courts located in Los Angeles County, California.

We reserve the right to update or modify these Terms at any time. Changes will be posted on this page with an updated effective date. Your continued use of our website or Services after any changes constitutes acceptance of the revised Terms. The terms in effect at the time of your booking will govern that transaction.
